How do I enroll or review my benefits?
Log on by entering your Last Name (as it appears on your paycheck, excluding any hyphens or apostrophes), or your login name (if not your last name), and your Social Security Number (or password - the site is secure) in the fields at the top of the page. This will give you access to view and/or print your detailed benefits and enroll if you are eligible. You may also call 1-800-866-2325 for a printed version of your benefits.

How do I change my address or beneficiary?
It is important to update your address so that mailings and other information can be successfully delivered to you. To change your address, log into this website and choose View/Modify Your Enrollment Information. To change beneficiaries, log into this website and choose the option to Assign or Change Beneficiaries.

Why haven't I received a Prescription Card?
You will receive only one ID card. You will use this one card for medical, prescriptions, vision, & dental.

What if I lose my ID card?
Contact Beal Benefits at 1-800-866-2325. We will be happy to order a replacement card and issue you a temporary card to use until the new permanent card arrives in 10-12 business days. You can also log into the enrollment site, (instructions above) and print ID cards for Dental or Health benefits.

How do I find a list of Vision providers?
Please click Locate a Medical or Dental Provider link to your left. Depending on the provider, you may be asked to pay for the services up front and you will then need to file a claim for your reimbursement.

Do I need to use certain Dentists?
Please click on the Locate a Medical or Dental Provider link to your left. Depending on the provider, you may be asked to pay for the services up front and you will then need to file a claim for your reimbursement.

How do I file a claim?
You may print a claim form from this website by choosing the homepage option of Claim and Misc Forms. Complete the top section of the form and attach an itemized billing. Mail the claim to the address listed on your ID card. Most In Network providers will NOT require a claim form, once you've shown them your Health or Dental ID card.

How do I use the mail order prescription option?
You may print a mail order claim form from this website by choosing the homepage option of Claim and Misc Forms. Choose the form for the mail order service listed on your benefit summary. Follow the instructions to fill out this form and include a copy of your prescription when mailing to the address of the form. Payment can be made in the form of check, money order, or fill out the credit card information on the form. Prescriptions will be mailed to you in 2-3 weeks.

What if I'm sick and it's after office hours?
Always contact your physician first. Even if it is after regular office hours, the physician will have an answering service that can contact the physician on call. Your physician's office may refer you to the Emergency Room or Urgent Care.

Do I need a referral from my regular physician to see a specialist?
No. You may go directly to a specialist without a referral. However, you do want to keep in mind there are lesser benefits if you see an out of network specialist versus a PPO provider. You can search for in network providers at this website's home page; just click on the find a provider link on the left side of the home page.

How can I find my login information?
Your last name (or login name) and social security number (or password), by default, are used to log into this site. If you have logged in previously and have changed your login to a new username/password, click here to retrieve these.

How do I know I'm safe entering personal information on this site?
The communication between your web browser and this web site is encrypted, using the latest technology available. Features of a secure web site are:
